Send-A-Note

You can now send a personalized note to a loved one at Extendicare. Simply choose an appropriate note card design, type in your message and add photos if you want. You can choose "Happy Birthday", "Thinking of you", or "Get well soon". These cards will be frequently updated for your convenience.

Fill out the form with patient's name, facility and date the card is to be delivered. Attach your card and message and press SEND. If you want to send additional pictures, see below.
